Our Process
2. Qualify
When you arrive at our office, you will be asked to fill out a short medical history form and some additional documents. Patients must have a valid Florida ID or Drivers license, or paperwork, to establish proof of residency in the state of Florida.
3. Meet with the Doctor
During the appointment, the Doctor will be going over your medical history. i.e. What is your condition? How long has it been going on? Was it caused from a situation or event? Have you taken prescriptions for it? Etc.
If the Doctor determines you qualify for a written medical cannabis recommendation, you will be registered with Florida’s Office of Compassionate Use, and receive your State Medical Marijuana Patient number, in our office before you leave.
4. State Registration and ID Card
You will receive an email from the Florida Dept. of Health after your appointment. Follow the email instructions. After following the email instructions, you will receive a second email with your “temporary” patient ID card with same-day approval for new patients with a Florida drivers license. Recertification or Transfer patients are active right after their appointment with the doctor.
